Patient update: Introduction of new triage system – June 2026

We would like to inform patients that from June 2026, Carden Surgery will be introducing a new patient triage system for GP appointments.

This change will ensure that all patients are assessed and directed to the most appropriate clinician within a suitable timeframe, based on their clinical need.

By adopting this approach, there will no longer be a need to call the surgery first thing in the morning, as all patients will have equal access to care regardless of when they make contact.

We understand that this change may be difficult for some patients who are used to traditional ways of accessing appointments; however, this is being introduced to help improve access for everyone.

Whether you contact us by phone, in person, or online, the same information will need to be provided to ensure you receive the most appropriate appointment. For this reason, using our online link will be the most efficient way to contact the surgery.

*Appointments with our practice nursing team will continue to be booked following our current system.

Important transition information

To ensure a smooth transition to the new system, we will not be booking any routine appointments after 29th May. This is to support a safe and effective move onto our new triage platform.

What will change?

  • Patients requesting a GP appointment will need to complete a triage form. If this is done over the phone or at the surgery, this will involve providing the same details required online to our reception team. We understand that some patients are uncomfortable sharing information with our teams; however, we will be unable to triage any requests without the required information.
  • Once your triage information has been submitted, it will be reviewed by a clinician, and you will be contacted by our reception team to arrange the most clinically appropriate appointment.
